The coalition members meet and strategize approaches to making Stanislaus County an “asthma friendly” community by promoting awareness, education, management, and prevention. General meetings will be held quarterly on Wednesdays in the Martin Conference Room.

Breastfeeding Promotion Coalition

Framework for a Thriving Stanislaus

The 2010 - 2020 Framework for a Thriving Stanislaus is a plan created by a large group of individuals and public and private organizations to coordinate community efforts to improve the health of our county and well being of our citizens. The Framework for a Thriving Stanislaus is a living plan, which means it is constantly being updated as needed. We encourage you to return to this page to see how the plan develops as we make progress on achieving the goals.

The Framework for a Thriving Stanislaus is organized into four areas that impact community prosperity and well-being, also known as broad determinants of health, which are social, economic, infrastructural and environmental factors that influence health, well-being and prosperity. These are:

HEART Coalition

The HEART Coalition was convened in 1999 due to the high rates of heart disease in Stanislaus County. The coalition’s mission is to reduce cardiovascular disease risk factors to improve the health status and quality of life of the residents of Stanislaus County. The Coalition has sponsored several projects in an effort to fight the obesity epidemic and reduce heart disease. Visit the Coalitions website at www.heartcoalition.org, to learn more. Immunization Coalition

A group of private and public organizations, agencies, business leaders and concerned citizens working together to protect infants and young children against vaccine preventable diseases. Meetings are held quarterly on the 1st Thursday at 12 noon to 1 pm at the Stanislaus County Health Services Agency, BHRS Redwood Room.

Maternal Adolescent Health Advisory Committee

A multidisciplinary group that shares the goal of assisting women to have healthy, full term infant births. The group meets regularly and has developed strategies to reduce the incidence of low birth weight babies born in our county.

Mobilizing for Action Through Planning and Partnerships

Multidisciplinary representatives in the Local Public Health System use a continuous strategic planning process to improve the Health of Stanislaus County residents.

Promotora Network Meetings

A workforce and information sharing meeting exclusively for those who work in promotora or community health worker capacity. These meetings are held in Spanish only and usually take place the last Wednesday of the month from 10-12.

Teen Pregnancy Prevention Collaborative

The Teen Pregnancy Prevention Collaborative is comprised of various community members and organizations. Organizations consist of Stanislaus County Office of Education, local Family Resource Centers, local school districts, Project YES, Ceres Partnership for Healthy Children, Planned Parenthood, and SHADE Teen clinic.

The Collaborative mission statement is: "To promote health relationships, sexuality, and lifestyles among teenagers and young adults in Stanislaus County".

The goal of the Collaborative is: "To ensure that youth in Stanislaus County receive comprehensive sex education and have easy access to confidential free or low cost family planning services".

The Collaborative meets quarterly (March, June, Sept., & Dec.) on the first Wednesday of the month from 3:00pm - 4:30pm. Please call for meeting locations. The Collaborative welcomes any member of the community to participate and attend meetings.

Tobacco-control Outreach and Prevention Services Coalition

The mission of the TOPS Coalition of Stanislaus County is to lead the community in the development and promotion of tobacco control outreach and prevention services. Quarterly meetings are held the first Wednesday of the month in March, June, September and December from 11:30am-1:00pm at the Sutter Gould Medical Foundation, Health Education and Conference Center, located at 1700 McHenry Ave Suite 60B - Modesto.
