(MAPP) website.
We hope that this will allow you easy access
to essential information pertaining to the MAPP
process as it is currently being implemented
in Stanislaus County.
To sum it up in a few words,
Mapp is looking at ways of improving health
at all levels, including:
Community level (community residents &
community based organizations)
Completed -- Analysis
of data collected for Phase II of the Community
Health Assessment (Community Health Status
Assessment). Phase I of the Community Health
Status Assessment is complete. This information
is available by visiting the HSA website.
Completed -- Consolidation
of reports related to the Community Themes
and Strengths Assessment
Completed -- Consolidation
of reports related t the Force of Change Assessment
Completed -- After
approval by Senior Management the Local Public
Health System Assessment will begin. This
will include a three different group meeting
a various points during a day scheduled to
complete an assessment tool developed by CDC
and NACCHO with members of the local public
healthy system.
In Progress --
Desired completion of the MAPP process and
implementation of the Action Cycle.
