Child Health & Disability Prevention (CHDP)

The CHDP exam is a FREE well-child (head to toe) physical and screening.

Types of EXAMS offered:

• Well baby/well child
• Preschool / Head Start
• 1st Grade School Exam
• Sports or Camp Physical
• Teen Physical

All of the above exams include:

• Health and developmental history
• Complete head to toe physical examination
• Oral Health Assessment
• Nutritional Assessment
• Vision Screening
• Hearing Screening
• Screening tests for anemia, blood lead, tuberculosis, urine abnormalities, sexually transmitted diseases, and other problems as needed
• Immunizations as appropriate for Age Recommended Immunization Schedule for Persons Aged 0–6 / 7–18 Years

At time of the health assessment, the CHDP provider makes referrals as needed to:

• Women, Infants and Children (WIC) Nutrition
• Dentists for preventive or restoration care
• Specialty health care providers as necessary
• Mental health care
• Other resources as needed
