The CHDP exam is a FREE well-child (head to toe)
physical and screening.
Types of EXAMS offered:
Well baby/well child
Preschool / Head Start
1st Grade School Exam
Sports or Camp Physical
Teen Physical
All of the above exams include:
Health and developmental history
Complete head to toe physical examination
Oral Health Assessment
Nutritional Assessment
Vision Screening
Hearing Screening
Screening tests for anemia, blood lead, tuberculosis, urine abnormalities, sexually transmitted diseases, and other problems as needed
Immunizations as appropriate for Age Recommended Immunization Schedule
for Persons Aged
0–6
/
7–18 Years

At time of the health assessment, the CHDP provider makes referrals as needed to:
Women, Infants and Children (WIC) Nutrition
Dentists for preventive or restoration care
Specialty health care providers as necessary
Mental health care
Other resources as needed
