HEART Coalition of Stanislaus County[HEART]

This is a community partnership of health care providers, education, businesses, health plans, and concerned citizens. Its mission is to reduce cardiovascular disease risk factors to improve the health status and quality of life of residents of Stanislaus County.

Coalition committees are:

• Program
• Data

Coalition and its committees meet on a monthly basis to plan for and implement CVD prevention projects and activities for the community.

Implemented projects:

• Healthy heart public information fair
• CVD prevention resource guide
• Website development

Activities being planned:
