Adult Health Case Management[AH] |
| Public Health Nurse Case Management services.
Goal:
- Coordination of services to prevent or delay hospitalization
and promote appropriate use of medical system
- Comprehensive nursing assessment
- Case plan development with client and care givers
- Referrals to community health and social services
- Assistance and advocacy in accessing services.
Target Population:
Adult residents of Stanislaus County who are medically
frail, technology dependent or have multiple medical
diagnoses. |
