High Risk Maternal and Child Health[HRMCH]

 

Public Health Nurse Case Management Services

Goals:

  • Increased parent/care give knowledge of needs of medically high risk infants
  • Prevention of rehospitalization of medically high risk infants.
  • Improved health outcomes for medically high risk infants and children through improved medication and treatment compliance and care by parent or gaurdian.
  • Families and care givers will have increased knowledge of appropriate parenting and child care.
  • Families and care givers will know of community resources appropriate for their needs.

Services:

  • Comprehensive assessment of infant/childs physical condition, growth and development, environment and resource needs.
  • Assessment of care givers parenting skills and knowledge and understanding of childs condition, treatments, medication and care.
  • Development of a case plan with input from family/care giver and other providers.
  • Referrals to community resources.
  • Coordination with medical providers.
  • Education about infant/child care, parenting and specific medical condition.

Target Population: Infants and children with serious health concerns, with a medical diagnoses, with alterations in growth and development. Family and/or caregiver needing parenting/childcare education, resources.
