Perinatal Outreach and Education [POE]

Our team of Community Health Workers reaches out with information on health related services and resources available

 

  • You see them going door to door in the community
  • You see them at community events and health fairs.

They assist families to access and apply for Medi Cal,Healthy Families, Healthy Cubs, Access for Infants and Mothers (AIM)

They speak: Spanish, Cambodian, Laotian, and Hmong

EDUCATION
Public Health Nurses and Community Health Workers provide presentations to promote healthy pregnancies and healthy children.

Some of the Topics: Fetal Development, Prenatal Care, Pregnancy, What can Harm a Developing Fetus, Smoking and Pregnancy, Post Partum Depression.

Target Population: Women of childbearing age; their families; pregnant women; those who care for and provide services to pregnant women.
