Vital Records is moving from its current location at 108 Campus Way in Modesto to a new location on the central campus of the Health Services Agency. Vital Records will stay open at the Campus Way location during the move and there will not be a disruption of service.

Effective March 4, 2002, the new location of Vital Records will be at 820 Scenic Drive, Room 27 in the Public Health Building. Hours of service will remain the same, Monday-Friday 8 a.m. to 5 p.m. The phone number remains the same at (209) 558-8070.

The Stanislaus County Health Services Agency (HSA) is an outpatient medical system with 9 medical offices located throughout Stanislaus County.
