What CHDP offers families:
The CHDP child wellness exam is a NO COST (head to toe) physical and screening.
Types of EXAMS offered:
- Well baby/well child
- Preschool / Head Start
- 1st Grade School Exam
- Sports or Camp Physical
- Teen Physical
All of the above exams include:
- Health and developmental history
- Complete head to toe physical examination
- Oral Health Assessment
- Nutritional Assessment
- Vision Screening
- Hearing Screening
- Screening tests for anemia, blood lead, tuberculosis, urine abnormalities, sexually transmitted diseases, and other problems as needed
- Immunizations as appropriate for Age Recommended Immunization Schedule for Persons Aged 0–6 / 7–18 Years

At time of the health assessment, the CHDP provider makes referrals as needed to:
- Women, Infants and Children (WIC) Nutrition
- Dentists for preventive or restoration care
- Specialty health care providers as necessary
- Mental health care
- Other resources as needed
