Who We Are
The Stanislaus County Immunization Program provides immunization services to the families of our community.
- Our mission is to promote community health by providing vaccines to prevent diseases and improve vaccine coverage in our community.
What We Do
The Immunization Program offers free or low-cost immunization services to eligible children and adults in Stanislaus County. We educate and provide information about immunizations, immunization schedules, childcare and school laws, immunization registries, and records. We also collaborate with community partners, including non-profits, healthcare providers, and school districts. Flu vaccine clinics are conducted each fall for all age groups within the county.
The following individuals are eligible to receive immunizations at 1524 McHenry Ave, Modesto CA 95350:
- Children and adults who do not have insurance
- Children and adults whose health insurance does not include vaccines
- Persons 0-18 years who are Alaskan Native or Native-American
- Persons 0-18 years who have Medi-Cal or are Medi-Cal eligible
- Persons 6 months and older in need of influenza (flu) vaccine regardless of health coverage
Individuals are not eligible for vaccines at Stanislaus County Immunization Clinic if their insurance includes vaccination coverage, even if there are co-pays or deductibles. What We Offer
Vaccination is the safest way to protect against many preventable diseases and their complications. Vaccines control infectious diseases by reducing or preventing their transmission through multiple means. Therefore, it is important to follow up with your healthcare provider to check if you are up to date on your immunizations.

- Haemophilus influenza type b (Hib)
- Varicella (chickenpox)

- HPV (Gardasil)
- Polio
- Shingles (Zostavax) for adults 50 & over
- Rotavirus
- Influenza (Flu)
- COVID-19 (Pfizer and Moderna)
How to make an appointment:

What to bring to appointment:
- Picture identification of parent or legal guardian
- Health insurance
- Bring your child’s immunization record (yellow card)
- Letter from school (stating what immunizations are missing)
- Child’s birth certificate or passport
Request a Copy of Immunization Record or COVID-19 Vaccination
- Only immunization records reported to Regional Immunization Data Exchange (RIDE) / Healthy Futures will be available
- A fee will be charged for each child or adult immunization record copy
- A copy of COVID-19 vaccination printout will be given at no cost upon request
